I bought this spoon about 40 years ago. I presumed it was American and still believe it is. Unfortunately I have never identified the maker. The mark is obviously hard to read. My best guess is that it is it is DR or BR over a Fleur de Lys. Either letter could be a D, a B, or an R so the only way to identify the maker, would be to find the same mark on another piece and then make an attribution. I presume if you are reading this you know that both heart shaped cartouches and fleur de lys are found in early American silver. I would date the spoon to c. 1730.
Needless to say I have checked other anglophone countries, The Channel Islands, provincial Scotland, etc. and have not found the mark there either, which does suggest the possibility of an American origin.
